The State is currently conducting a dependent eligibility verification
project. Every employee who has the family coverage must send in
documentation to prove your dependents are still eligible. The State
mailed letters to all affected employees in July. They will send a final
letter on August 6.
Please make sure you turn in the documentation as soon as possible. If
you do not turn in the documentation by September 4, 2009, your dependents
will lose their health insurance coverage.
Note: If you use your 2008 income taxes as documentation, you may black
out all social security numbers and financial data. Make sure to write
the Identification Number at the top of each page you submit. This number
is located on the top, right side of the verification form.
