Kindergarten Registration

Things to bring to register your child for Kindergarten:

Child’s:

1.     Birth Certificate

2.     Social Security Card

3.     Physical- has to be within past 6 months of starting school    

4.     Immunization Record

** Record of physical and immunizations are usually documented on a        “Green Form” that can be obtained by your child’s physician.

If you do not have your child’s Social Security Card:

·        Call (423) 581-0258: They can mail you an application.

·        Go to 2438 E. A.J. Hwy: Morristown, to get a copy  Hours:  M-F    9a.m. – 4 p.m.

·        Fill out an application online at www.ssa.gov

If you do not have your child’s Birth Certificate:  (There are two locations where you can go to obtain an official copy.

·        Jefferson County Health Department: (865) 397-3930

Hours: M-F   8 a.m. – 4 p.m.

·        Knox County Health Department:  (865) 215-5000

Hours:  M-W-F from 8 a.m.- 4 p.m. & T-Th from 8 a.m.- 6:30 p.m.

·        Hamblen County Health Department:  (423) 586-6431

Hours:  Friday from 8a.m.-9a.m. & 1p.m.-2p.m.

** The parent of the child must be the one requesting the Birth Certificate.

Cost:  $7.00 for a short form.
